AFTER SCHOOL ENRICHMENT CLASS REGISTRATION FORM

ADVANCE REGISTRATION IS A MUST!! 

You may mail registration form with tuition or come by in person. No registration after 2nd class meeting.  The office is open Monday through Friday at 8:00 a.m.  If advance registration is not sufficient, class may be cancelled.  Should a class cancel due to insufficient enrollment, every effort will be made to notify you and you will be given an opportunity to select another class or receive a full refund.  If Decatur City Schools are closed because of inclement weather, the Community Education classes will be cancelled.  Cancellations will be announced on local television and radio stations.

(Please notice:  Only tuition for Baton and Karate is paid to Decatur City Schools Community Education.  Tuition for Gymnastics is paid to Head Over Heels.  Head over Heels registration forms are available at Community Education or in the school office.  Call 552-3067 for further information!)

A RESERVATION CANNOT BE GUARANTEED UNTIL TUITION IS PAID.

Refund Policy:  Full refund if requested by 12:00 p.m. the day of the 1st class meeting; partial refund if requested before 2nd class meeting, except for classes made under special circumstances.  No refunds after 2nd class meeting.

NO OBSERVERS ARE ALLOWED IN ANY CLASSES.
